How many drainagesessions?
The most asked question, and the one where answers vary most. Here are the benchmarks that come up in most protocols.
The short answer: between 8 and 12 sessions over six to eight weeks, for most body contouring surgeries. The long answer is worth reading, because the total matters far less than when the sessions happen.
Why drainage after surgery
Liposuction, a tummy tuck or a BBL cross the fat layer and damage part of the lymphatic network along the way — those very fine channels that carry fluid out of the tissue and back into circulation. The body responds by sending fluid into the area — that's the swelling — and that fluid takes time to leave, because the exit route is precisely what was disrupted.
Manual lymphatic drainage is a set of slow, very light, rhythmic strokes that follow the path of those channels and encourage that fluid toward the areas where the network still works.
This is not deep tissue work. If it hurts, it isn't drainage.
When to start
Common protocols place the first session 24 to 72 hours after surgery, and some surgeons prefer day 3 to 5. The variation comes from different schools, the type of surgery and whether drains are in place.
What doesn't vary: the surgeon clears it. Nobody else. A therapist who advises starting without that clearance is not acting in the client's interest.
Frequency, week by week
The most widespread pattern:
- Weeks 1 and 2 — two to three sessions a week. This is the densest and most useful window. Some protocols go as far as every other day in week one.
- Weeks 3 and 4 — one to two sessions a week, as swelling comes down.
- Weeks 5 to 8 — one session a week, with more tissue work and less pure drainage.
- After that — every two weeks, then as needed.
Roughly: 4 to 6 sessions in month one, 3 to 5 in month two, plus optional maintenance. Hence the familiar 8 to 12.
What moves the number
- The extent of the surgery. A single-area lipo is nothing like 360 lipo combined with a BBL.
- When care begins. Starting on day 4 and starting in week three don't need the same number of sessions — or the same kind of work.
- The body. Two women operated on the same day by the same surgeon don't swell the same way. It's frustrating and it's how it is.
- Consistency with compression. A garment worn half the time cancels out part of the benefit of the sessions.
What about a late start?
Plenty of women arrive at week four or six having done nothing, often because they had surgery abroad and nobody mentioned follow-up.
It isn't lost. The work simply changes character: there's less fluid to move and more firm tissue to soften, which usually means more sessions and slower progress. It's never too late to take on established fibrosis.
How long is a session
Allow 60 to 90 minutes for a full post-operative drainage session, and about 30 minutes more if it's given at home, for setup and packing up.
Is it covered by insurance?
Often, yes — when the drainage is given by a therapist who is a member of a recognised association such as the RMPQ, and the group plan covers massage therapy. Amounts and annual caps vary a great deal between plans.
Call the insurer before the first session. Five minutes on the phone beats a surprise at the end of the path.
